The Independent Investigations Office (IIO) launched an investigation after Vancouver Police Department (VPD) notified the agency regarding the death of an inmate at VPD jail. 

In a statement the VPD said,  "At approximately 8:00 a.m. on June 15, staff at the VPD Jail discovered a prisoner unconscious. BC Emergency Health Services was called immediately and attended. The man, who is not being identified, was pronounced deceased by EHS."

 The statement further read, "The VPD notified the IIO, which has now taken over the investigation."
